Case Management & Housing Navigation
Through our community partners, we provide case management and housing navigation to help individuals access resources, set goals, and secure stable, supportive housing.
Support may include topics like:

Assessment of individual needs and strengths

Personalized goal-setting and action planning

Assistance with securing safe and affordable housing

Connection to employment and education resources

Support accessing healthcare and mental health services

Guidance on benefits and financial assistance programs

Advocacy with landlords, agencies, and service providers

Ongoing follow-up to maintain stability and prevent setbacks
Therapeutic Services
Through our outside referrals, licensed professionals provide counseling and psychotherapy services for individuals, both in person and online. Each session is conducted using a secure, user-friendly videoconferencing platform that meets HIPAA and federal security standards, ensuring privacy and compliance.
